Why one-off, not a subscription.
Health insurance is renewed once a year. You actually use Covered three or four times - when you join a new policy, when you're booking a procedure, when something gets denied, at renewal. Charging monthly for that pattern is brutal for users and creates churn we don't want to optimise against.
